About

A physician who has stayed in Birgunj

Dr. Niraj Kumar Singh has spent his entire clinical career in Nepal’s Madhesh Province — first in the emergency room and intensive care unit, later as a consultant physician, and throughout as a voice for the district’s health infrastructure.

Dr. Singh began practising at what was then Narayani Sub-Regional Hospital in 2001. Between 2003 and 2009 he served as Emergency In-charge and as ICU/CCU In-charge — the years that shaped how he works: quickly, carefully, and with the assumption that a patient may not be able to explain everything themselves.

He left Birgunj to complete postgraduate training in internal medicine in Kathmandu, then returned. He is today a consultant physician in the Department of Internal Medicine at Narayani Hospital, and sees patients privately at Manakamana Health Clinic on Railway Road.

Pandemic service

He stayed on the ward

Birgunj sits on Nepal’s busiest land crossing with India, which made it one of the country’s first COVID-19 hotspots and kept it among the worst affected. Dr. Singh worked through both major waves inside Narayani Hospital.

Fifty large oxygen cylinders a day, and patients still arriving too late.

Through 2020 and 2021 he was one of the few clinicians in Province 2 who kept talking to the press on the record — not about himself, but about what the hospital was running out of. In August 2020 he warned that patients were arriving too late to save, and that the province did not have the isolation beds, ventilators or oxygen plants to meet what was coming.

By April 2021 he was reporting that Narayani was treating patients past its normal capacity and burning through roughly fifty large oxygen cylinders a day. The Nepal Medical Association appointed him its Province 2 coordinator for the pandemic medical response.

In July 2021 the Barbara Foundation recognised him in the doctors’ category of its COVID-19 Distinguished Award, at a ceremony held under the words “Thank You COVID Warriors”. Later that year his name appeared on the Government of Nepal’s Constitution Day honours list.

Clinical focus

What Dr. Singh treats

Dr. Singh practises general internal medicine, including diabetes, thyroid disorders, hypertension, chronic obstructive pulmonary disease and acute febrile illness. The notes below describe each area in plain language.

General Internal Medicine

The diagnosis and long-term management of illness in adults — from infections, fevers and chest problems to blood pressure, anaemia and the conditions that arrive together rather than one at a time.

Who this is for

Adults with a new symptom that has not been explained, a long-term condition that needs reviewing, or several conditions that interact with one another.

M.D. in Internal Medicine; consultant in the Department of Internal Medicine, Narayani Hospital.

Diabetes Care

Assessment and ongoing management of type 2 and type 1 diabetes: blood sugar control, medication review, and watching for the complications that develop quietly — in the eyes, kidneys, nerves and heart.

Who this is for

People newly diagnosed with diabetes, those whose blood sugar has been difficult to control, and anyone with a family history who wants to be checked.

Postgraduate training in diabetes (ADA, USA); practises as a diabetologist.

Thyroid Disorders

Investigation and treatment of an underactive or overactive thyroid — a gland whose malfunction is easy to miss, because tiredness, weight change and low mood are so often attributed to something else.

Who this is for

People with unexplained fatigue, weight change, hair or skin changes, irregular periods, or a family history of thyroid disease.

Named in Dr. Singh’s own clinical summary; co-author of a 2024 study on thyroid disorders in polycystic ovary syndrome.

Hypertension (B.P.)

Assessment and ongoing management of high blood pressure, including reviewing readings, medicines and related risks to the heart, brain and kidneys.

Who this is for

People with repeatedly raised blood-pressure readings, an existing hypertension diagnosis, or treatment that needs review.

Confirmed by the practice; managed within Dr. Singh’s general internal medicine work.

Chronic Obstructive Pulmonary Disease (COPD)

Assessment and long-term management of COPD, a chronic lung condition that can cause persistent cough, phlegm, wheezing and breathlessness.

Who this is for

People with diagnosed COPD, persistent breathing symptoms, or a change in symptoms that needs clinical review.

Confirmed by the practice; managed within Dr. Singh’s general internal medicine work.

Acute Febrile Illness (AFI)

Clinical assessment of a new fever or fever-related illness to identify the likely cause, check for warning signs and decide what testing or treatment is appropriate.

Who this is for

People with a recent fever, chills, body aches or other new symptoms who need a medical assessment rather than self-diagnosis.

Confirmed by the practice; managed within Dr. Singh’s general internal medicine work.

In his own words

Philosophy of care

I believe every patient deserves compassionate, respectful, and evidence-based care. My approach is to listen carefully, explain clearly, and treat the whole person — not just the illness. I aim to help patients achieve better health through prevention, timely treatment, and a trusted partnership built on honesty and dignity.

Questions

Frequently asked

How do I book an appointment?

Where is the clinic?

Manakamana Health Clinic is on Railway Road at Amar Chowk, in Birgunj-4, Parsa. The “Get directions” button on this page opens the exact location in Google Maps.

What are the consultation hours?

Monday to Friday, consultations are available from 8:00 AM to 9:00 AM and from 3:00 PM to 7:00 PM. On Saturday and Sunday, consultations are available from 10:00 AM to 3:00 PM.

What conditions does Dr. Singh treat?

He practises general internal medicine for adults, with particular attention to diabetes and thyroid disorders. If you are unsure whether your problem is one he can help with, describe it briefly when you contact the clinic and you will be told honestly.

What should I bring to my appointment?

Bring any previous prescriptions, laboratory or imaging reports, and the medicines you are currently taking — the actual boxes or strips are more useful than a list from memory. If another doctor has referred you, bring the referral letter.

Can I see Dr. Singh at Narayani Hospital instead?

Dr. Singh is a consultant physician in the Department of Internal Medicine at Narayani Hospital, which is a government hospital with its own outpatient registration process. That process is managed by the hospital, not by this clinic.

What happens at a first consultation?

Dr. Singh will ask about your symptoms and your history, examine you, and explain what he thinks is going on. If tests are needed he will say which ones and why. You should leave understanding both the plan and the reason for it — if you do not, ask.

What should I do in an emergency?

Do not wait for an appointment and do not use this website. Go directly to the nearest hospital emergency department. In Birgunj, the emergency department at Narayani Hospital operates around the clock.
